Financial Assistance for Children/Families

A Board of Trustees governs the Foundation and is responsible for establishing the policies for assistance, expenses, or donations consistent with the stated purpose for which The Foundation was established. The Grant Committee of the Board of Trustees will consider requests for assistance/expenses from all eligible children and/or families and/or committed to using the funds within The Foundation’s mission. Children/Families can only receive funs every three (3) years from the Hope Foundation.
The H.F.M.V. will support assistance, but is not limited to, providing money for treatments, drug/medication costs, transportation to local or distant medical facilities, (if abled to be billed directly to Foundation) and/or various household expenses (within reason), dedicating its resources to the education and/or research across the Mahoning Valley that provides services for chronically/terminally ill children & their families. All expenses must be paid directly to the organization for the good and/or service.
The foundation will not cover; house mortgages/rent; utilities (electric, gas, etc); any cost covered by insurance programs or third-party carriers; costs covered by Medicare, Medicaid, or medical assistance; and costs not directly related to treatment of diagnosis or diseases.
To be considered for funding, a family/child must meet the following criteria and submit the application online:
- The request must meet the mission of the H.F.M.V. to serve chronic or terminally ill children throughout the Mahoning Valley (Trumbull, Mahoning, & Columbiana receive first priority).
- Submit a written request through the form, detailing how funds will be used.
- A copy of the Legal Guardians Tax Returns from the previous year.
- Financial Assistance Packet Application (includes Assistance Application, Monthly Expense Report, & HIPPA/Release Form and any additional costs you might have during the month or year). All applications must be submitted online.
- A Signed Letter from child’s primary physician with diagnosis on letterhead.
To be considered for assistance, applications must be received by the H.F.M.V. by the date on the Grants Application of the Hope Foundation website. All applications received after the deadline will not be accepted!
The H.F.M.V. Grant Committee will review applications, and awards will be issued at the Committee’s recommendation in amounts determined by the Board of Trustees. The maximum assistance that can be awarded to a children/family is up to $2,500. (Exceptions may be made for emergency and unusual circumstances approved by the Executive Director or Board of Trustees of the Foundation). If funding is received, a follow-up report must be provided to The Foundation documenting how the funds were used within six (6) months of the date that the funds were given. At the discretion of the Board of Trustees, the HFMV reserves the right to recover any funds that were not used for the purpose as stated in the assistance application or appear to have been misused.
